Hospital Discharge Social Worker Role at Ackerman Pierce

Job Summary

We are seeking a skilled and experienced Qualified Social Worker to join our Hospital Discharge team in Somerset Council. The ideal candidate will have post-qualifying experience working within a Qualified Social Worker - Hospital Discharge team within the UK Social Care industry.

Responsibilities

The successful candidate will be responsible for:

  • Working with vulnerable adults in a hospital setting
  • Preparing and submitting reports and assessments
  • Developing and implementing care plans
  • Managing complex cases
  • Liaising with other professional bodies
  • Ensuring safe and timely discharges for service users
  • Supporting self-directed support and personal budgets
Requirements

The minimum requirements for this role include:

  • HCPC registration
  • One year of post-qualifying experience within the UK Social Care industry
  • Enhanced DBS/CRB clearance (or eligibility for Ackerman Pierce to obtain)
  • Eligibility to work in the UK
