Discharge to Assess Care Assistant

We are seeking a dedicated Discharge to Access (D2A) Care Assistant to join our clients team in Somerset and the surrounding areas. As a D2A Care Assistant, you will play a pivotal role in assisting customers with their rehabilitation needs in the comfort of their own homes.
Key Responsibilities:- Provide Safe and Reliable Support: Deliver tailored support to each customer's individual rehabilitation programme, ensuring their safety and well-being.
- Collaborate with Care Teams: Work closely with the care team, hospital discharge team, and occupational therapist to assess and meet the rehabilitation needs of customers.
- Respect Customer Dignity: Ensure care is delivered with informed consent, respecting and promoting the dignity of each customer.
- Monitor Progress: Regularly monitor and report customer progress towards achieving rehabilitation goals in a professional and timely manner.
- Effective Communication: Ensure clear communication with cluster groups of fellow professionals to aid customers' rehabilitation.
- Design Rehabilitation Plans: Work under the guidance of the occupational therapist to design and enhance person-centred rehabilitation plans.
- Assist with Daily Tasks: Assist customers with personal care, medication management, meal planning and preparation, housework, and more.
- Use Aids and Equipment: Safely use aids and equipment while respecting customers' dignity.
- Engage Customers: Engage customers in social, physical, and mental activities to promote well-being and independence.
- Record Customer Information: Record and report all relevant customer information, changes in condition, emergencies, and safeguarding matters.
- Maintain Confidentiality: Maintain confidentiality of customer and family information.
- Competitive Salary: Enjoy a competitive annual salary of £26,000.
- Travel Allowance: Receive a travel allowance of 35p per mile.
