Hospital Discharge Specialist

Ackerman Pierce is seeking a skilled Hospital Discharge Advanced Practitioner to join their team in Somerset. As a key member of the Hospital Discharge team, you will play a vital role in supporting adults in their community to remain independent for as long as possible.
Key Responsibilities:- Deliver High-Quality Care: Provide high-quality, joined-up social care, health, and wellbeing services to enhance rather than replace existing informal support networks.
- Support Hospital Discharge: Support people out of hospitals in a timely manner and prevent admissions by utilizing community assets to enable them to return home.
- Empower Individuals: Give people control of the care and support services they receive, ensuring they are delivered where, when, and by the people they want, and achieve the outcomes that are important for them.
- HCPC Registration: Must be registered with the Health and Care Professions Council.
- Experience: 1-2 years post-qualifying experience within the UK Social Care industry.
- DBS/CRB: Enhanced DBS/CRB required (Ackerman Pierce can obtain for you).
- Eligibility: Eligibility to work in the UK.
- Personal Consultant: Your own dedicated consultant.
- Access to Social Care Jobs: Access to a variety of Social Care jobs nationwide.
- DBS and Compliance Service: DBS and compliance services provided.
