Health and Wellbeing Support Professional

We are seeking a skilled Health and Wellbeing Support Worker to join our team at North Kerrier East Primary Care Network. As a key member of our team, you will work closely with GPs, Mental Health Practitioners, and Care Coordinators to support practice patients in managing their physical and mental health conditions.
Key Responsibilities- Manage and prioritize your own caseload to support individuals in taking proactive steps to improve their health and wellbeing.
- Use coaching skills and motivational interviewing to empower individuals, their families, and carers to identify their goals and develop personalized care plans.
- Work collaboratively with multidisciplinary teams to effectively manage complex cases and coordinate input from various professionals and services.
- Support individuals in accessing self-management education courses, peer support, and other interventions to improve their health and wellbeing.
- Deliver personalized care plans by coordinating input from a range of professionals and services, and helping individuals and their carers navigate across health and social care services.

- Good level of education
- Enrolled in or undertaking or qualified in an appropriate health coaching/motivational interviewing training course
- A suitable and relevant qualification for working with adults and/or children
- Higher level qualification such as NVQ in adult care Level 3 or qualification relevant to health or social care or a relevant qualification in working with children
- Experience of working in primary care and GP practices
- Experience or training in Mental Health Suicide Awareness for adults and children
- Understanding how to apply health coaching in group settings and working in groups
- Experience or training in personalized care and support planning
