Key Responsibilities- Work collaboratively with clinical teams to identify patients who require proactive care planning and coordination.
- Develop and implement personalized care plans, working closely with patients, families, and carers to ensure their needs are met.
- Coordinate care between healthcare professionals, services, and organizations to ensure seamless transitions and effective care delivery.
- Provide excellent communication and interpersonal skills, adapting to the needs of diverse patients and stakeholders.
- Utilize technology and data to inform care planning and coordination, ensuring accurate and timely data collection and reporting.
- Work closely with the PCN Business Manager and team to ensure effective administration and coordination of care services.
We are looking for an individual with:
- Good level of education and relevant qualifications.
- Proven experience in health, social care, or related fields, with a strong focus on patient-centered care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to analyze and interpret data, with a focus on improving patient outcomes.
- Strong organizational and planning skills, with the ability to prioritize tasks and meet deadlines.
