Job Title: Community Wellbeing Professional

We are seeking a highly skilled Community Wellbeing Professional to join our team at North Lincolnshire Council. As a key member of our Adult Care Services, you will play a vital role in supporting individuals with learning disabilities and complex needs.

About the Role

In this exciting opportunity, you will have the chance to work closely with individuals, their families, and carers to understand their needs and develop personalized support plans. You will also have the opportunity to contribute to the development of new policies and procedures to improve adult care services.

As a Community Wellbeing Professional, you will work collaboratively with other teams, including social workers, Approved Mental Health Professionals, and senior social workers, to ensure that individuals receive comprehensive and coordinated support.

Key Responsibilities
  • Support individuals with learning disabilities and complex needs through assessments and support planning
  • Develop and maintain effective relationships with individuals, families, and carers
  • Contribute to the development of new policies and procedures to improve adult care services
  • Work collaboratively with other teams to provide comprehensive and coordinated support
Requirements
  • Experience of working in adult care services or a related field
  • Knowledge of the Care Act 2014 and the Mental Capacity Act 2005
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
Salary and Benefits

The salary for this position is £23,836 - £30,478 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits including pension scheme, annual leave, and flexible working arrangements.
