Senior Mental Health Practitioner - Band 6

**Job Summary**

We are seeking an experienced and resilient clinical practitioner to join our Crisis Resolution Home Treatment Team at Harplands Hospital, Stoke-on-Trent. As a Senior Mental Health Practitioner, you will be responsible for delivering high-quality care and supervising others in the team.

**Key Responsibilities**

* Participate in the assessment of care needs and develop, implement, and evaluate individual care plans
* Liaise with the multi-disciplinary team and outside agencies to implement patient care
* Ensure care activities are research-based and aligned with changes in the field of activity
* Assist in setting and maintaining standards of care and participate in effective management of the team

**Requirements**

* Registered mental health practitioner with a degree in nursing, social work, or occupational therapy or equivalent experience
* Strong communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
