We are seeking a highly motivated and experienced Mental Health Practitioner to join our Newcastle Adult Community Mental Health Team at the Lyme Brook Centre. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals in our local community.
Key Responsibilities- Conduct assessments and triage referrals to ensure timely and effective interventions.
- Work collaboratively with our multi-disciplinary team to develop and implement individualized care plans.
- Provide support and interventions to service users, including home visits and follow-up appointments.
- Liaise with other professionals to ensure high-quality, gold-standard care and support.
We are looking for a qualified and experienced Mental Health Practitioner who possesses:
- A diploma-level qualification in Mental Health Nursing, Occupational Therapy, or Social Work.
- Experience of working therapeutically in community settings.
- Knowledge of various approaches to mental health assessment.
