Job Title: Older Adults Mental Health Practitioner

We are seeking a highly skilled and compassionate Older Adults Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our High Peak Older Adult Community Mental Health Team, you will play a vital role in delivering high-quality care to older adults with functional mental health conditions.

About the Role:

This is an exciting opportunity to work in a dynamic and supportive team environment, where you will have the chance to make a real difference in the lives of our patients. As an Older Adults Mental Health Practitioner, you will be responsible for providing comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs. You will build therapeutic relationships, develop care plans, and empower individuals to take control of their health.

You will work collaboratively with colleagues, key partners, and the local community to develop services that meet the mental health needs of the community. This will involve undertaking comprehensive assessments, developing care plans, and providing support to individuals to improve their mental health.

Key Responsibilities:
  • Provide comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs.
  • Build therapeutic relationships, develop care plans, and empower individuals to take control of their health.
  • Work collaboratively with colleagues, key partners, and the local community to develop services that meet the mental health needs of the community.
  • Undertake comprehensive assessments, develop care plans, and provide support to individuals to improve their mental health.
Person Specification:

We are looking for an experienced and skilled Older Adults Mental Health Practitioner who possesses the following qualities:

  • Relevant qualification in mental health nursing or occupational therapy.
  • Post-qualification experience in the relevant field.
  • Experience of working with older adults with functional mental health diagnoses.
  • Ability to work independently and as part of a team.
  • Good verbal and non-verbal communication skills.
  • Good IT skills.
