Job Title: Older Adults Mental Health Practitioner

We are seeking a highly skilled and compassionate Older Adults Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust.

About the Role

This is an exciting opportunity to work with our High Peak Older Adult Community Mental Health Team (CMHT) and Living Well Team, providing support to older adults with functional mental health conditions in a community setting.

As an Older Adults Mental Health Practitioner, you will be responsible for delivering a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality.

You will work collaboratively with colleagues, key partners, and those within the local community to develop services that meet the mental health needs of the community.

Key Responsibilities
  • Undertake comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs.
  • Build therapeutic relationships, develop a plan of care with individuals, and assist them through their difficult times by empowering and enabling them to take control over their health.
  • Support and motivate individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources.
  • Maintain relationships with community partners, including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations, and the local communities we serve.
Person Specification

We are looking for an individual with:

  • A relevant qualification for the post (Diploma in Nursing/Degree in Mental Health Nursing) or Degree in Occupational Therapy.
  • Post-qualification experience in the relevant field, including experience of working with older people with functional mental health diagnoses.
  • Good verbal and non-verbal communication skills, as well as good IT skills.
  • The ability to work independently, as well as part of a team.
