About the Role

We are seeking an enthusiastic and experienced Band 6 Older Adult Specialist to join our High Peak Older Adult Community Mental Health Team (CMHT) and Living Well Team in Swadlincote.

Main Responsibilities
  • Support the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality.
  • Work collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
  • Employ a wide range of skills, building on and developing expertise, and actively seeking advice and collaboration with issues that may not be readily within their sphere of practice.
Key Requirements
  • Relevant qualification for the post (Diploma in Nursing/Degree in Mental Health Nursing) or Degree in Occupational Therapy.
  • Experience in working with Older people with functional mental health diagnoses.
  • Experience of community working and ability to travel across the geographical area.
  • Able to work independently, as well as part of a team, and meet the travel requirements of the post.
