Job Summary

We are seeking a highly motivated and experienced Social Care Practitioner to join our Adult Social Work Long Term Team in Derbyshire County Council. The successful candidate will be responsible for supporting adults over 18 and their carers, as well as transition cases from the age of 14.

Key Responsibilities

  • Assess and review service user needs in the community and residential settings.
  • Work within budgets and have knowledge of outcomes-focused assessments and reviews.
  • Collaborate with other professionals, service users, and their families and carers in a multi-disciplinary partnership way.
  • Create person-centred and strengths-based assessments and support plans.
  • Commission services to enable individuals to meet their outcomes, promoting a home-first culture.

Requirements

  • Experience and understanding of assessing and reviewing service user needs.
  • Ability to work in a fast-paced and challenging environment.
  • Strong communication and interpersonal skills.
  • Ability to travel and have access to own transport (mileage allowance payable).
