Mental Health Practitioner PositionThis position offers the opportunity to work in a dynamic team, providing care and support to individuals experiencing mental health crisis in their own homes.
Main Responsibilities- Conduct thorough assessments to inform treatment plans
- Work closely with community teams to ensure consistent and knowledgeable care
- Develop and implement risk management plans to ensure safe and effective care
- Professional registration: NMC, Social Work England, HCPC
- Relevant experience in urgent care and/or community
- Excellent risk assessment and management skills
- Estimated salary: £45,000 - £55,000 per annum, depending on experience
- 27 days annual leave plus bank holidays rising to 29 after 5 years and 33 days after 10 years

This is a full-time position, working on a rota basis including some weekends, evenings, and nights when the service requires.
