The Role
This is an exciting opportunity for an experienced Occupational Therapist to join our Community Crisis Response Team. As a key member of the team, you will provide high-quality care to patients in their homes, avoiding hospital admissions. You will work closely with the multidisciplinary team to assess, plan, and deliver care that meets the needs of each patient.
Key Responsibilities
- Assess patients' needs and develop individualised care plans.
- Deliver care that promotes independence, dignity, and well-being.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
- Maintain accurate records and reports on patient progress and outcomes.
Requirements
- Minimum 2 years post-graduate clinical experience.
- Autonomous practitioner with experience of managing a caseload.
- Experience of teaching and rehabilitation.
- Ability to work across a broad range of clinical areas and presentations.
Salary and Benefits
The salary for this role is £35,000 - £45,000 per annum, depending on experience. We also offer a comprehensive benefits package, including a pension scheme, life assurance, and annual leave.
