Carer Support Navigator

We are a dynamic team based in Manchester, working to improve hospital discharge processes and support carers. As a Care Coordinator Specialist, you will be part of a collaborative team that works closely with health teams, discharge teams, and local services.
Responsibilities- Develop and implement safe discharge planning and information for patients and unpaid carers
- Embed procedures to raise awareness of the needs of carers in a secondary care setting
- Empower carers to be involved in the discharge process
- Experience of working in a healthcare or social care setting
- Strong communication and interpersonal skills
- Knowledge of hospital discharge processes and carer support services
The salary for this role is £35,000 - £40,000 per annum, depending on experience. We offer a range of benefits, including a pension scheme, life assurance, and annual leave.
