Job Summary:

We are looking for an experienced Paediatric Nurse to take on the role of Paediatric Case Manager. This is an exciting opportunity to join our team and make a real difference in the lives of our clients.

In this role, you will be responsible for delivering complex care packages to children in Buckinghamshire, Oxford, and West Berkshire ICB. You will work closely with a team of carers, providing training and support to ensure they have the necessary skills and knowledge to deliver high-quality care.

  • Designing and implementing care plans that meet the unique needs of each client
  • Providing ongoing monitoring and support to ensure the highest level of care is delivered
  • Collaborating with community teams, CHC nurses, and CCG commissioners to develop and maintain effective care packages

The ideal candidate will have a strong background in paediatric nursing, with a minimum of 2 years' experience in the UK. You will also need to have a supportive management style, up-to-date experience of tracheostomy care and ventilators, and a full UK driving licence.
