Respiratory Nurse Specialist
Job Summary
We are seeking a highly skilled Respiratory Nurse Specialist to join our team at Brownlow Group Practice. As a Specialist Practitioner, you will work across the four pillars of advanced practice, leading and delivering Nurse-led respiratory clinics for patients in our Brownlow Central practice.
The focus will be on Asthma management within our large student population, as well as review of patients with other respiratory conditions such as COPD, Bronchiectasis, and Pulmonary Fibrosis. You will also provide support to our homeless population with bi-monthly clinics.
As a key member of our team, you will work autonomously to proactively target high-risk patients, providing an assessment of their needs and an appropriate management plan. You will have a high level of expertise and experience in this field, with the ability to manage complex cases, including first-contact assessment, examination, and management of patients presenting with acute respiratory problems.
You will provide support and mentorship to our less experienced Practice nurses, while also supporting education to the wider team. Additionally, you will work with the practice nursing team to contribute to data gathering, as needed, to achieve the Practice's fulfillment of the Asthma and COPD targets within the Quality Outcomes Framework.
Key Responsibilities
- Deliver Nurse-led respiratory clinics to support the Practice Nurse team
- In line with targets within the Quality Outcomes Framework and NICE guidelines, review patients with asthma and COPD using relevant EMIS clinical templates to ensure assessments are completed and coded to meet QOF requirements
- Use non-medical prescribing to review medications, including maintenance therapy and acute self-management plans
- Assess inhaler technique and optimize inhaler therapy, giving consideration to greener respiratory care, such as low-carbon inhalers, overuse, and overprescribing
- Provide a written personalized asthma/COPD action plan
- Review patients admitted to an emergency care setting with an acute asthma attack or exacerbation of COPD within 2 working days of discharge
- Review patients admitted to hospital with symptoms of breathlessness related to any other underlying respiratory problem or LRTI
- Ensure patients coded COPD have had spirometry performed to confirm diagnosis
- Ability to interpret spirometry testing as required
- First-contact assessment, examination, and management of patients presenting with acute respiratory problems, including potential COVID-19 symptoms
- Review patients diagnosed/suspected with long COVID
- Administration of relevant health prevention vaccinations
- Provide home visits to ensure our housebound respiratory patients receive a timely review
- Request and interpret relevant blood tests and investigations
- Build links with and liaise with secondary care and community services
- Work within national guidelines, policies, and procedures, as well as local protocols, supporting the development of these in-house as needed to ensure the patient receives the highest quality evidence-based care
- Maintain effective communication with patients, carers, relatives, and members of the multidisciplinary team, ensuring information is shared appropriately
