About the Role

We are seeking a skilled Clinical Transition Specialist to join our team at South West London and St George's Mental Health NHS Trust. As a Clinical Transition Specialist, you will play a vital role in supporting the transition of young people from child and adolescent mental health services to adult services.

The successful candidate will have a strong background in clinical work, with experience of working with adolescents and their families. You will be responsible for providing leadership on safeguarding issues, contributing to specialist assessments, and developing services to meet the needs of young people transitioning to adult services.

About the Salary

The estimated salary for this role is £45,000 - £60,000 per annum, depending on experience.

About the Location

This role will be based in Richmond, London, with easy access to public transport links.

Job Description

Key Responsibilities:

  • Provide leadership on safeguarding issues to CAMHS teams for cases in process of transitions
  • Contribute to specialist assessments of young children and their parents/carer referred to the services
  • Support evaluation and decision-making process for transition and treatment interventions
  • Exercise autonomous professional responsibility contributing towards assessments, discharge and transition of young people to adult mental health services
  • Contribute to risk assessment(s) and risk management(s) for young children and parents/carer
  • Communicate in a skilled and sensitive manner, information concerning the assessment, formulation and treatment plans of young children and their parents/carer
