About the Role

We are looking for a registered nurse with experience in supporting and promoting patient experience, particularly among young people with health conditions. The successful candidate will work closely with our Roald Dahl Clinical Nurse Specialist for teenagers and young adults to build on existing work and develop new initiatives that support healthcare transition and developmentally appropriate care.

Main Responsibilities
  • Develop and implement effective healthcare transition pathways that meet the needs of young people living with health conditions.
  • Collaborate with colleagues from across children's and adults' services to ensure continuity of care.
  • Work with voluntary organisations that provide excellent support for young people and their families.
  • Advocate for young people's needs and promote their involvement in healthcare decision-making.
  • Contribute to the development of policies and procedures that support healthcare transition and patient-centered care.
Required Skills and Qualifications
  • Registered nurse with a 1st degree or equivalent mentorship course.
  • Up-to-date registration with the NMC.
  • Experience of working with young people with health conditions and supporting patient experience.
  • Excellent communication and project management skills.
  • Able to work independently and as part of a team.
