Key Responsibilities
  • Implement evidence-based practice to manage patients with chronic diseases
  • Develop and implement health promotion and wellbeing programs
  • Assess patient needs and provide appropriate healthcare services
  • Manage wound care, including leg ulcer management and compression therapy
  • Prescribe medication and review patient medications for enhanced compliance
  • Coordinate travel medicine services and request pathology services as required
  • Oversee the provision of chronic disease management and refer patients to secondary/specialist care as needed
  • Ensure accurate coding of medical records and maintain chronic disease registers
  • Develop and implement wellbeing programs for patients
