Key Responsibilities- Implement evidence-based practice to manage patients with chronic diseases
- Develop and implement health promotion and wellbeing programs
- Assess patient needs and provide appropriate healthcare services
- Manage wound care, including leg ulcer management and compression therapy
- Prescribe medication and review patient medications for enhanced compliance
- Coordinate travel medicine services and request pathology services as required
- Oversee the provision of chronic disease management and refer patients to secondary/specialist care as needed
- Ensure accurate coding of medical records and maintain chronic disease registers
- Develop and implement wellbeing programs for patients
- Prioritize health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognize, assess, and refer patients presenting with mental health needs
- Implement vaccination programs for adults and children
- Liaise with external services/agencies to ensure patient support
- Delegate clinical responsibilities safely and effectively
- Support the clinical team with safeguarding matters
- Monitor and ensure safe storage, rotation, and disposal of vaccines, medicines, and consumables
- Support junior members of the nursing team
- Participate in shared learning and local initiatives to enhance service delivery and patient care
- Registered Nurse and member of the NMC
- Wound Care qualification
- Phlebotomy qualification
- Vaccinations qualification
- Desirable qualifications: Experience using EMIS, Diabetes qualification, Asthma/COPD qualification, Travel Health qualification, Immunisation qualification, Qualified smear taker
