Job Title: Practice Nurse

We are seeking a highly skilled and experienced Practice Nurse to join our dynamic team at Park Road Group Practice. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

About the Role:

The successful candidate will be responsible for:

  • Implementing evidence-based practice to manage patients with long-term conditions
  • Providing nursing services to our practice population, ensuring effective delivery of care
  • Supporting the practice management team in reviewing and delivering clinical policies and procedures
  • Developing and implementing health promotion and wellbeing programmes
  • Assessing patient needs and providing appropriate healthcare, incorporating evidence-based practice
  • Managing wound care, including leg ulcer management, compression, and Doppler
  • Prescribing medication for patients in accordance with PGDs and own scope of practice
  • Reviewing patient medications to enhance compliance, within own scope of practice
  • Coordinating the provision of travel medicine services
  • Requesting pathology services and processing pathology results as required
  • Overseeing the provision of chronic disease care, delivering patient care as necessary, and referring patients to secondary/specialist care as required
  • Ensuring coding of medical records is used effectively
  • Ensuring the appropriate maintenance and use of chronic disease registers
Person Specification:

The ideal candidate will possess:

  • Registered Nurse and member of the NMC
  • Wound Care
  • Phlebotomy
  • Vaccinations
  • Desirable qualifications include experience using EMIS, Diabetes qualification, Asthma/COPD qualification, Travel Health, and Immunisation qualification including knowledge of child immunisation programme
