Community Nursing Specialist

Livewell Southwest CIC is seeking a skilled Community Registered Nurse to join our team. As a Community Registered Nurse, you will play a vital role in coordinating care for individuals through evidence-based best practice, in accordance with National and Organisational approved policies and procedures.
Main Responsibilities
As a Community Registered Nurse, your key responsibilities will include:
- Providing holistic, evidence-based care to individuals in their homes or community settings.
- Working collaboratively with multidisciplinary teams to ensure seamless care coordination.
- Developing and implementing individualized care plans that meet the unique needs of each patient.
- Providing education and support to patients and their families to promote healthy living and independence.
- Participating in quality improvement initiatives to enhance patient outcomes and experiences.

Person Specification
We are looking for a skilled and compassionate Community Registered Nurse who possesses the following qualities:
Essential
- Evidence of recent updating or training in nursing and healthcare.
- Current knowledge of developments in nursing and healthcare.
- Understanding of Clinical Governance, including audit and research.
- Understanding of clinical supervision and continued professional development.
- Understanding of team function.
- Awareness of recent Government policies.
- Understanding of Community Nursing services and local issues.
Desirable
- Teaching qualification.
- Mentorship or equivalent.
- Relevant post-registration courses or knowledge base relevant to the post.
- Non-Medical Prescribing.
