Job Title: Anticoagulation Clinical Nurse Specialist

As a Clinical Nurse Specialist in Anticoagulation, you will play a vital role in managing daily anticoagulation nurse-led clinics, receiving direct referrals from General Practitioners and Consultants, and coordinating all investigations and treatment for DVT patients from referral to diagnosis according to the DVT protocol.

You will participate in approved Patient Group directions by the Trust, appropriate to our sphere of practice, to maintain safe anticoagulation for patients. Additionally, you will coordinate patient discharge, liaising with all members of the multidisciplinary team to ensure that safe delivery of anticoagulation continues as an outpatient.

Key Responsibilities

You will carry out programmes of care and treatment determined by others, provide advice in relation to the care of individual or groups of patients, and deliver education programmes to patients, carers, and other nursing staff as agreed by the Clinical Nurse Specialist.

Person Specification

We are looking for a highly skilled and experienced Clinical Nurse Specialist with:

  • Two years at Band 5 or equivalent
  • Evidence of continuing professional development
  • Demonstrates the use of evidence-based practice in the implementation of change
  • Experience within the speciality
Qualifications

We require:

  • RGN / RSCN / RM
  • Willing to work towards a post-grad qualification in speciality
  • Teaching qualification
Skills and Abilities

You will need to demonstrate:

  • Evidence of ability and experience in communicating across a diverse range of people and organisations
  • Evidence of ability to function with a high degree of personal accountability and autonomy
  • Effective leadership skills
