We are seeking a motivated and decisive Social Care Support Officer to work within the Short Term Support Service in Adult Services at Lancashire County Council.

Key Responsibilities
  • Work closely with a multitude of services to ensure the best possible outcomes for individuals in hospital settings and at home.
  • Take a person-centred and strengths-based approach to support service users.
  • Assess necessary care and provide superior holistic assessments.
  • Support with early discharges from hospital and visit people's homes to assess various situations.
  • Identify any and all risks to prevent hospital admission or any future health-related issues.
Requirements
  • Passion for making a difference to people's lives.
  • Ability to make tough decisions and remain firm but fair when determining interventions.
  • Insightful and aware of resources and services around you to mitigate risks.
  • Get up and go attitude and ability to think on your feet.
  • Resilience and ability to work independently and as part of a team.
