About the Job

Join our dynamic Acute Clinical Team in Bridgend as an Advanced Clinical Practitioner/Trainee Clinical Practitioner. In this exciting role, you will contribute to the development of our community healthcare services, working closely with a multi-disciplinary team to deliver high-quality patient care.

With a focus on prevention, early intervention, and integrated care, we aim to make a tangible difference in the lives of our patients and communities. As a key member of our team, you will be responsible for providing specialist knowledge and advanced practice skills to patients, as well as collaborating with other healthcare professionals to achieve excellent patient outcomes.

Main Responsibilities:
