About the Role
This is an exciting opportunity to join our team as a Staff Nurse in the Intermediate Frailty Service. As a Community Staff Nurse, you will be working closely with other healthcare professionals to provide high-quality care to patients in their own homes. With a focus on frailty crisis response and acute clinical management, you will be responsible for conducting comprehensive clinical assessments and monitoring of patients.

The successful candidate will have a strong clinical knowledge base, excellent communication and interpersonal skills, and the ability to work effectively as part of a team. They will also be committed to ongoing professional development and have a passion for delivering high-quality patient care.

- Competitive salary: £31,383 - £43,041 per annum, depending on experience

Your Qualifications and Experience
- UK NMC Registered Nurse qualification
- Experience working in a multidisciplinary team setting
