Job Title: Advanced Community Health Specialist

Cwm Taf Morgannwg University Health Board provides primary, secondary, and community health and wellbeing services to around 450,000 people. As an Advanced Community Health Specialist, you will be responsible for delivering the Healthy Child Wales Programme and promoting the health and wellbeing of children and families.

Main Responsibilities:
- Assess health need and implement health-enhancing activities for pre-school children and families within a defined geographic area or General Practice population.
- Generate information to influence policies affecting health.
- Plan and transfer care of children, young people, and their families within a defined caseload.
- Manage a defined health visiting caseload.
