Salary: £22,000 - £27,000 per annum
Key Responsibilities:
Deliver high-quality care to clients in their own homes or community settings
Conduct comprehensive assessments of clients' needs and develop individualized care plans
Work collaboratively with healthcare professionals to ensure seamless care delivery
Provide education and support to families and caregivers
Requirements:
- Nursing qualification (RGN/RMN)
- Experience in community care
- Excellent communication and interpersonal skills
