Job Title: Community-Based Care Lead - Occupational Therapy

We are seeking an experienced Occupational Therapist to join our team as a Community-Based Care Lead. In this role, you will be responsible for managing the clinical delivery of care within Acute Care at Home and providing urgent holistic assessments of unwell elderly patients at home.
About the Role:
- Manage the clinical delivery of care within Acute Care at Home
- Provide urgent holistic assessments of unwell elderly patients at home
- Work alongside an Acute Care Practitioner to respond to falls and sick patients in the community
