Nursing Team Lead Specialist

We are seeking a highly skilled Nursing Team Lead Specialist to join our team at Tripple Tree Ltd. The successful candidate will be responsible for leading a team of specialist nurses in the provision of high-quality care to patients with long-term conditions.
The role will involve:
- Care Planning: Developing, implementing, and evaluating care plans that address the expressed health, social, and cultural needs of patients.
- Clinical Leadership: Providing clinical leadership to nursing teams to enable them to develop approaches that address the needs of patients with complex long-term conditions.
- Caseload Management: Coordinating the monitoring of patient care, disease management, and supporting clinical staff to ensure continuity and continuing care.
The ideal candidate will have experience in leading teams, strong communication skills, and a passion for delivering high-quality patient care. They will also have a good understanding of NICE guidelines and local targets.
The salary for this role is estimated to be around £45,000-£55,000 per annum, depending on experience.
