About the Role

We are seeking a skilled Community Nursing Specialist to join our team in the Waterlooville locality.

This part-time position involves working with patients in their own homes, aiming to prevent hospital admission and maintain independence. You will be responsible for routine visits and responding to Urgent Crisis Response calls, collaborating with SCAS and other members of the Multidisciplinary Team.

The successful candidate will participate in ICT/MDT meetings, coordinate care plans, and work autonomously within their scope of practice as a registered practitioner.

Main Responsibilities
  • Manage a defined community caseload via Nursing, Physiotherapy, or Occupational Therapy interventions for a group of GP Practices as part of the Integrated Care Team.
