Key Responsibilities:

  • Conduct assessments under the Care Act 2014 to identify the needs of young adults transitioning to adult services

  • Facilitate access to services that support young adults in achieving their goals and aspirations
  • Work collaboratively with other professionals to ensure a seamless transition from children's services to adult services

Requirements:

  • Qualified Social Worker with experience working with young adults in transition
  • Strong assessment and planning skills, with the ability to develop and implement effective care plans
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with other professionals
  • Knowledge of the Care Act 2014 and its application in adult services
