About the Role:We are seeking a dedicated and experienced Qualified Social Worker to join our team, who is passionate about working with young people and their families to provide support and guidance.
The role is unique in its close working relationships with Children's Services, Education, and Health, ensuring that children transitioning to Adult Services receive the right support from the right people as they prepare for adulthood.
Key Responsibilities:- Conduct Care Act 2014 compliant eligibility assessments for young people and their carers, identifying eligible social care needs and developing focussed, person-centred, and strengths-based outcomes within support plans.
- Monitor, review, and maintain up-to-date support plans, ensuring they meet the changing needs of young people and reflect good value for money, while advocating for young people's rights, choices, and access to services.
- Safeguard and promote the welfare of young people, adhering to relevant legislation and policies, and identifying and assessing levels of risk and need in complex situations.
- Collaborate with multidisciplinary teams, including children's services, education, healthcare professionals, and community partners, to enable a holistic approach to meeting the needs of young people.
- Experience dealing with sensitive situations and strong interpersonal and communication skills.
- Experience of Conducting Care Act 2014 and dealing with Children's Services.
- Social work qualification and at least one year's post-qualifying experience in a social work role, meeting the requirements of the social work regulation, with the exception of Newly Qualified Social Workers (ASYE).

We offer a hybrid model of working, with the flexibility to work from home and office, with a requirement of 2 office-based days per week, recognising the benefit of a balanced work-life approach.
