We are seeking a permanent, dedicated, and experienced Qualified Social Worker to join our dynamic and supportive team in Adult Services.
As a Qualified Social Worker, you will have the opportunity to make a real difference in the lives of young people with additional needs and their families.
The role is unique in its close working relationships with Children s Services, Education, and Health to ensure that children transitioning to Adult Services receive the right support from the right people as they prepare for adulthood.
Key Responsibilities
- Conducting Care Act 2014 compliant eligibility assessments for young people and their carers in a timely manner to identify eligible social care needs and developing focused, person-centered, and strengths-based outcomes within support plans.
- Maintaining up-to-date support plans, ensuring they meet the changing needs of young people and reflect good value for money while advocating for young people s rights, choices, and access to services.
- Safeguarding and promoting the welfare of young people, adhering to relevant legislation and policies, identifying and assessing levels of risk and need in complex situations, and undertaking safeguarding investigations and delivering protective and/or supportive services for individuals at risk.
- Collaborating with multidisciplinary teams, including children s services, education, healthcare professionals, and community partners, to enable a holistic approach to meeting the needs of young people.
Requirements
- Experience dealing with sensitive situations and strong interpersonal and communication skills.
- Experience conducting Care Act 2014 assessments and dealing with Children s Services.
- A Social Work qualification and at least one year s post-qualifying experience in a social work role, meeting the requirements of the Social Work Regulation, except for Newly Qualified Social Workers (ASYE).
