We are seeking a dedicated Mental Health Practitioner to join our integrated Access Team in North East Lincolnshire. As an experienced professional, you will work closely with our multidisciplinary team to provide high-quality care and support to individuals experiencing mental health crises.

About the Role

This exciting opportunity is ideal for those passionate about delivering person-centred, trauma-informed care. You will be responsible for initial assessments, implementing holistic biopsychosocial interventions, and developing clear care pathways. Your ability to manage competing priorities and exercise sound clinical judgment will be invaluable in this role.

Key Responsibilities
  • Conduct thorough risk assessments and develop effective safety plans
  • Provide telephone support and crisis intervention services to clients, carers, and external agencies
  • Work collaboratively with community-based services and resources to ensure seamless care coordination
  • Develop and implement care packages in various settings, prioritizing individual needs
  • Participate in research projects and audit activities to improve service quality
Requirements

To succeed in this challenging yet rewarding role, you must possess:

  • A current professional qualification (UK RMN/RNMH/SW or equivalent)
  • Evidence of postgraduate training/studies
  • Two years' post-registration experience working with severe and enduring mental illness in an acute or crisis setting
  • Advanced skills in risk assessment, communication, and liaison with various stakeholders
