Job Overview

We are seeking a highly motivated and solution-focused individual to join our team as an Independent Living Coordinator. This role will involve working with older people who have had recent hospital admissions to help them maintain their independence at home.

The successful candidate will be a people person, highly organized, and skilled at practical problem-solving. They will be passionate about maximizing older people's income and relish working in a constantly evolving environment.

As an Independent Living Coordinator, you will facilitate access to services, resources, and facilities, manage a caseload of customers, and respond to high levels of demand for our services. You will also receive a bespoke induction and training to equip you with the knowledge and skills to complete holistic assessments and provide solutions to identified issues.

Key Responsibilities:

  • Supporting older people to maintain their independence at home
  • Facilitating access to services, resources, and facilities
  • Managing a caseload of customers and responding to high levels of demand
  • Completing holistic assessments and providing solutions to identified issues

Requirements:

  • Degree in a relevant field or equivalent experience
  • Excellent communication and interpersonal skills
  • Highly organized and able to prioritize tasks effectively
  • Able to work in a fast-paced environment and adapt to changing circumstances
