We're seeking a dedicated Care Specialist to join our team, working closely with Sheffield City Council to promote ongoing enablement for individuals receiving care.
The Role:
You will conduct comprehensive assessments and continuous reviews of care needs, ensuring that care plans are dynamic and continually evolving to meet changing needs and preferences.
Key Responsibilities:
- Support providers in fulfilling their duties under CQC regulations.
- Identify and implement the least intrusive services to boost independence.
- Proactively address life challenges to maintain quality of life.
- Collaborate with local Primary and Community Health Care networks, and Community Services.
What We're Looking For:
To be considered for this role, you must have:
- A good understanding of the current legislative framework in reference to adult social care.
- Excellent communication and interpersonal skills.
- The ability to work effectively in a fast-paced environment.
Salary Information:
The estimated salary for this role is £30,000 per annum.
