Job Overview

We are seeking a highly skilled and organized Care Coordinator to join our team at University Hospitals Birmingham NHS Foundation Trust. In this role, you will play a crucial part in ensuring the smooth flow of patients through our elderly care wards.

About the Role

The successful candidate will be responsible for developing excellent internal communication pathways, utilizing the internal escalation process, and ensuring that the multi-disciplinary team adheres to the Department's professional standards.

You will work closely with staff at all levels, communicating effectively within the department/units, and deal proactively with all enquiries, welcome and orientate patients and their relatives/friends to the clinical area.

Main Responsibilities

  • Monitor the transport process and identify any delays or issues that the clinical team are experiencing.
  • Act as the first point of contact for users of the service and deal professionally with telephone enquiries, ensuring that the caller's needs are met or they are transferred to the most appropriate person.
  • Liaise with Site Team as and when appropriate, and ensure that all staff are aware of any information that will assist in any safe transfer/discharge.
  • Promote all transfers/discharges within agreed timeframes, identifying bottlenecks in the system which prevents the smooth and timely transfer of patients.

Requirements

  • A good general education (e.g., GCSE English and Maths A-C) is essential, along with Business Administration NVQ level 3 or equivalent experience in an administrative environment.
  • Experience of dealing with the public/customer service experience, working in a secretarial/administrative role with a proven track record of problem-solving, is also required.
  • You must be able to work under pressure, use your own initiative, and deal with unpredictable situations.
  • Able to travel to multiple sites, confident in dealing with people at all levels, and demonstrate an understanding of equality and diversity are essential skills for this role.

University Hospitals Birmingham NHS Foundation Trust offers a competitive salary range of £25,500 - £31,383 per annum, depending on experience, plus benefits including pension scheme, life assurance, and annual leave entitlement.
