Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Brook Street Social Care. As a Care Coordinator, you will be responsible for overseeing the delivery of home support services to clients in the Solihull area.
Key Responsibilities:- Conduct thorough assessments of new clients to identify their unique needs and preferences.
- Develop personalized care plans based on client assessments, outlining specific services and goals.
- Provide ongoing training and support to caregivers to ensure they are equipped to deliver high-quality care.
- Monitor the quality of care provided by caregivers through regular checks, feedback, and audits.
- Create and manage caregiver schedules to ensure optimal coverage and efficient service delivery.
- Respond to and resolve any crises or emergencies that may arise.
- Ensure compliance with all relevant regulations, policies, and procedures.
