About the Role

We are seeking a highly motivated and enthusiastic Support Worker to join our team at Kent & Medway NHS & Social Care Partnership Trust. As a Support Worker, you will play a vital role in providing support and care to individuals with mental health needs.

Key Responsibilities
  • Assess and plan care for service users, working closely with the multidisciplinary care team.
  • Liaise with Clinical Pathway Leads to deliver interventions and support service users.
  • Contribute to assessing clients referred to Mental Health Together or Mental Health Together Plus.
  • Work with individuals to facilitate access and engagement in community-based activities.
  • Deliver a high standard of support to service users and their carers, promoting their equality, dignity, and mental well-being.
Requirements
  • Previous experience in healthcare, preferably in mental health.
  • NVQ Level 2 in Care or Community Mental Health Care, with a commitment to undertaking a Level 3 qualification within a specified timeframe.
