Care Manager

We are seeking a highly motivated Care Manager to join our team at North Lincolnshire Council. As a key member of our Learning Disability and Sensory Work Team, you will be responsible for managing a caseload of complex cases and providing person-centred assessments and care plans.
About the RoleThis is a full-time position that requires experience of working directly with individuals who have learning disabilities and/or mental health needs. You will need to have a detailed knowledge of adult care and its underpinning legislation, as well as excellent problem-solving skills and effective communication and organisation skills.
The successful candidate will be able to work autonomously, demonstrate negotiation and IT system skills, and participate in liaising with other teams to ensure a good quality service.
Key responsibilities include:
- Managing a caseload of complex cases
- Liaising with internal and external teams
- Providing person-centred assessments and care plans
We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, a comprehensive induction, high standard of professional supervision, generous annual leave entitlement, and childcare salary sacrifice scheme.
