Dual Diagnosis Recovery Coordinator

Devon Partnership NHS Trust is seeking a highly skilled and dedicated individual to join our team as a Dual Diagnosis Recovery Coordinator. As a key member of our multidisciplinary team, you will play a vital role in supporting individuals with complex needs, including substance misuse and concurrent mental health issues.
Main Responsibilities- Manage a defined caseload of service users with a focus on those with a concurrent presentation of substance misuse and a severe and enduring mental health problem.
- Coordinate treatment packages that are personalized to the individual's treatment and recovery needs.
- Liaise with a wide range of statutory and non-statutory agencies to ensure effective engagement and treatment delivery.
- Provide evidence-based recovery-focused interventions to support individuals with complex issues related to their substance misuse.
- Develop links with mental health services and develop resources for the wider team.
