Dual Diagnosis Recovery Specialist

Devon Partnership NHS Trust is seeking a highly skilled and dedicated professional to join our team as a Dual Diagnosis Recovery Co-ordinator. This exciting opportunity will involve working with individuals who present with complex needs, including substance misuse and concurrent mental health issues.
Main Responsibilities- Provide recovery coordination for complex individuals presenting to TRI, including consideration of safeguarding needs, mental health comorbidity, and multi-agency risk assessment conferences.
- Monitor patients receiving substitute prescribing interventions to support their recovery for opiate misuse or misuse of other substances.
- Take responsibility for specific pathways and ensure relationships and communication are clear between Torbay recovery initiatives and primary care networks and mental health services.
- Support and engage people who typically find it hard to access and utilize community services and resources.
- Actively engage and work with individuals with a wide range of mental health problems, who may also have co-existing complex needs, including homelessness, substance misuse, significant risk to self or others, and social exclusion.
