Learning Disability Care Coordinator

Are you passionate about delivering high-quality care to individuals with complex needs? We are seeking a Learning Disability Care Coordinator to join our team. The salary for this position is approximately £35,000 - £45,000 per year, depending on experience.
About the RoleIn this role, you will work closely with patients, families, and other healthcare professionals to ensure that individuals receive the best possible care. As a Registered Mental Health Nurse or Learning Disability Nurse, you will use your clinical expertise to develop and implement individualised care plans that meet the unique needs of each patient.
Main Responsibilities- Develop and implement individualised care plans for patients
- Work closely with patients, families, and other healthcare professionals to ensure seamless care delivery
- Monitor and review patient progress, making adjustments to care plans as needed
To be successful in this role, you will need to have:
- A current NMC registration as a Registered Mental Health Nurse or Learning Disability Nurse
- An up-to-date knowledge of clinical practices and a strong understanding of CQC requirements and frameworks
- Experience of working with individuals with complex needs and developing effective care plans
