Complex Care Nurse
About the RoleWe are seeking a qualified RGN Nurse with excellent clinical skills in complex care to join our team as a Complex Care Nurse. As a key member of our Community Nursing Team, you will work closely with clients and their families to provide high-quality care and support.
Key Responsibilities- Conduct initial assessments in the community to identify client needs
- Train and assess workers' competencies, allocating support workers to patients as needed
- Work collaboratively with the Multi-Disciplinary Team (MDT) to ensure comprehensive care plans
- Attend meetings and make referrals as required
- Hold team meetings to discuss progress and address any concerns
- Qualified RGN Nurse with experience in complex care
- Excellent clinical skills, including peg feeds, ventilators, and tracheostomy management
- Able to travel across the North West region, requiring a car driver's license
- Competitive salary of £38,000 per annum
