About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Community Hub team at Central and North West London NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive assessments to identify individual needs and develop personalized treatment plans in collaboration with the multidisciplinary team.
  • Intervention and Support: Deliver evidence-based interventions, including individual and group therapy, to promote recovery and well-being.
  • Collaboration and Communication: Work closely with colleagues, families, and other stakeholders to ensure seamless care coordination and effective communication.
  • Risk Management: Systematically assess and manage risk to ensure the safety and well-being of service users.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date records of interventions, assessments, and treatment plans.
Requirements
  • Qualifications and Experience: Possess a relevant qualification in mental health nursing or a related field, with a minimum of 2 years' experience in a similar role.
  • Skills and Competencies: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Knowledge and Understanding: Possess a thorough understanding of mental health legislation, policies, and procedures, including the Mental Health Act 2007 and the NHS and Community Care Act 1990.
