We are seeking a highly skilled and motivated Occupational Therapist to work with our Urgent Community Response Team (UCR) in a dynamic and rewarding role.
Main Responsibilities- Deliver high-quality care to individuals at risk of hospital admission in their own homes, promoting independence and well-being.
- Collaborate with Consultant Practitioners, Advanced Clinical Practitioners, and other healthcare professionals to provide fast and effective services for patients with exacerbation of long-term conditions and/or frailty.
- Provide assessment and care planning, including history taking and physical assessment, without direct supervision, and delegate tasks to nursing and therapy colleagues as needed.

Person Specification- Qualified Occupational Therapist with HCPC registration and relevant post-registration experience in a variety of areas.
