Job Description

We are seeking a skilled and dedicated Wellbeing Support Worker to join our team at Bevan Community Benefit Society. As a Wellbeing Support Worker, you will play a vital role in supporting individuals with multiple health and social needs within a healthcare setting or community-based work with socially excluded groups.

The successful candidate will have a strong commitment to an enabling solution-focused approach to promote independence in service users. They will be able to develop trusting relationships with service users, giving them time to focus on their assets and what matters to them, and providing them with personalised help to take control of their health and wellbeing, live independently, and better understand the impacts of their lifestyle choices.

Key responsibilities include:

  • Undertaking holistic client needs assessments using a wellbeing template.
  • Supporting a caseload of clients for whom wellbeing support and mentorship might offer improved outcomes.
  • Proactively identifying people who would benefit from this type of help and managing and regularly reviewing your caseload to accommodate urgent referrals for support as required.
  • Escalating care back to the GP or other care professionals when the person's needs are beyond the scope of the health and wellbeing support worker.

We offer a competitive salary of £25,00030,000 per annum, depending on experience, plus excellent benefits and opportunities for professional development.
