Key Responsibilities:

  • Assess and understand the care and support needs of patients and their loved ones
  • Research and network across the system to identify available services and support
  • Work collaboratively with other healthcare professionals to develop and evaluate care management plans
  • Provide compassionate and person-centered care, prioritizing the needs of each individual
  • Line manage healthcare assistants and assess their clinical skills
  • Undertake staff observational and support visits in the community
  • Coordinate care and prioritize patient needs
