End of Life Care Specialist

Key Responsibilities:
- Assess and understand the care and support needs of patients and their loved ones
- Research and network across the system to identify available services and support
- Work collaboratively with other healthcare professionals to develop and evaluate care management plans
- Provide compassionate and person-centered care, prioritizing the needs of each individual
- Line manage healthcare assistants and assess their clinical skills
- Undertake staff observational and support visits in the community
- Coordinate care and prioritize patient needs
