About the Role
Job Summary
We are seeking a skilled and compassionate Registered Nurse to join our multidisciplinary team and contribute to providing holistic, person-centered end-of-life care. The successful candidate will assess and understand the care and support needs of patients and those important to them, working collaboratively with other healthcare professionals to improve the experience of dying, death, and bereavement.
Key Responsibilities
- Research and network across the system to understand the wide range of services available to patients and those important to them.
- Work with other providers to support the wider system in improving the experience of dying, death, and bereavement.
- Develop and evaluate care management plans alongside other healthcare professionals to meet the complex needs of patients and those important to them.
- Assess and understand the barriers and enablers to good communication and identify strategies to overcome challenges.
- Line manage healthcare assistants and assess their clinical skills.
- Coordinate care and prioritize patient needs.
